The law requires school personnel to report to the local public health agency individual cases of reportable diseases or conditions, as well as any suspected or confirmed disease outbreaks. Dental
All children entering their first year of school are required to submit a Kindergarten Oral Health Assessment (KOHA). Find information and downloadable forms regarding KOHA.
